For presumed meningioma based on MRI with plans for SRS, how often do you request a biopsy to confirm meningioma and WHO grade 1 disease?

 If not, then what is your preferred treatment and would you integrate SRS into it? 

Does this affect your subsequent imaging surveillance frequency?

Often physicians must choose between treatment with surgery or SRS. What patient factors would drive one to choose surgery or SRS?

With the publication of the ARUBA trail results, should management of AVMs shift toward medical management?  

Do you routinely offer re-irradiation? If so, what is your preferred dose and technique?

The literature does not seem to report on the rates of improvement of headaches.
