When is a bilateral bone marrow aspirate/biopsy indicated in the initial diagnosis of pediatric NHL patients?  

The NCI PDQ on pediatric NHL (https://www.cancer.gov/types/lymphoma/hp/child-nhl-treatment-pdq#_764_toc) indicates that bone marrow biopsy and aspiration should be performed as part of the diagnostic evaluation in these patients, but would you forego a bilateral bone marrow aspirate/biopsy in a patient with a very small localized NHL?
