Aside from ECE or positive surgical margins, when (if ever) would you offer post-operative chemoradiation in patients with squamous cell cancer of the head and neck?

The NCCN guidelines give the option of "consider systemic tx/RT" for "other factors," what do you consider as other factors? For example: close margin in a young patient with good PS etc.
