Do you add adjunctive gentamicin and/or rifampin for treatment of prosthetic valve Staphylococcus aureus endocarditis?
No, we do not add adjunctive gentamicin for treatment of Staphylococcus aureus (SA) prosthetic valve endocarditis (PVE). The potential benefit of using an aminoglycoside in this setting is minimal, if any, and is outweighed by the risk of toxicity.
With respect to using rifampin, it depends on whethe...
For Rifampin, it is often difficult to manage with my elderly population and all of their drug interactions. Apixaban is quite common in my people, and rifampi...
I would keep gentamicin for short duration until bacteremia resolved especially in patients without any kidney issues. And agree with others to use rifampin if no interactions or side effects.
We do not use gentamicin or rifampin. We published an article on whether rifampin should be used or not in PVE.
Our conclusions: The recommendations for using rifampin in the treatment of staphylococcal PVE vary among guidelines. The existing data supporting this recommendation is limited and primari...
There is no great evidence to support gentamicin and rifampin in staphylococcal prosthetic valve endocarditis. Academicians like me stick to evidence so I cannot support this practice.
