Yes, I would recommend pelvic RT to a patient with multiple risk factors, especially. It has been shown that stage II patients with substantial LVSI may have a higher risk of pelvic recurrence if treated with brachytherapy alone (Barnes et al., PMID 33485810).

Yes. I would offer external beam RT and brachytherapy. It is likely that as with breast cancer, it will take a very long follow up and huge numbers of patients to see a survival advantage.
