No, we do not routinely check G6PD levels prior to initiation of hydroxychloroquine (HCQ). The American College of Rheumatology does not recommend routine testing for G6PD prior to initiation of hydroxychloroquine (HCQ), but interestingly, package inserts often recommend caution in these patients.

I agree with @Dr. First Last’s excellent and detailed response with references. At NYU, we see no need for G6PD testing in patients starting hydroxychloroquine or chloroquine. There is a pharmacological explanation in that these are 4 aminoquinolones and it is the 8 aminoquinolones, such as primaqui...

We usually check CBC within 4 weeks of starting HCQ. Routine G6PD not done, however, if there is history of hemolytic anemia in the past then the test is ordered.
