When managing patients with ESKD who are about to be initiated on intermittent hemodialysis and have residual renal function, what approach do you take?

• I ensure not to remove extra fluid to avoid hypotension and subsequent renal injury, especially if the patient has a urine output of at least a liter per day
• I start with twice weekly hemodialysis, but I closely monitor residual renal function and educate patients about the potential need to increase treatment frequency in the future
