How does the presence of multifocal disease in early-stage breast cancer influence your treatment approach and risk stratification?

• Treat similarly to single-site lesions, using the size of the largest lesion for anatomic staging
• More likely to recommend chemotherapy if multifocality is present, regardless of the size of the largest tumor
• More likely to recommend adjuvant CDK4/6i if multifocality is present, regardless of the size of the largest tumor
• Recommend treatment that covers all identified subtypes if tumors vary in grade or receptor status
• Use genomic profiling to further stratify risk and tailor adjuvant therapy decisions
