I do not obtain an EEG unless something in the patient's history leads me to suspect seizures.

I'm all for the appropriate use of EEGs but I would say a basic EEG should be part of the evaluation. Ideally, a 24-hour ambulatory EEG (no video needed). At the very minimum, a routine EEG. I do it as a second-tier test except when specifically searching for conditions like CJD, NMDA encephalitis, or non-convulsive status. In these particular cases, I prioritize it as a first-tier test.
