Does delay to the time of lumpectomy impact your decision to omit radiation?
Would you consider omitting radiation in a elderly woman with ER-positive early stage breast cancer who was placed on 4 months of letrozole prior to lumpectomy?
The patient otherwise would have qualified for radiation omission but developed a large hematoma that developed after biopsy and delayed time to surgery

There seem to be two issues to consider in this situation. One is whether patients undergoing neoadjuvant endocrine therapy, then lumpectomy without RT, have an acceptable local recurrence rate. I would follow the same principle as I would use for low-risk ER-positive disease in those 65 and above.
