Does papillary thyroid cancer with extensive squamous differentiation require specialized treatment beyond RAI and TSH suppression?
Patient has 1.1 cm follicular variant PTC with extensive squamous differentiation. Neg 2/2 nodes. Already had total thyroidectomy. Would you recommend RAI or more tx like chemo due to squamous component?

Squamous differentiation associated with PTC represents an aggressive pathologic feature with poor long-term outcomes. I would consider this as a step towards dedifferentiation.
