For postoperative radiation for oral cavity cancer, should the entire surgical bed including grafts and plates (eg reconstructed mandible) receive the full 60 Gy?
The surgical bed created by reconstruction is often quite larger than the surgical bed from tumor resection and neck dissection.

Yes always. There are risks of tumor seeding or contamination during recon, or as a result of altered lymphatic and vascular drainage.
