What's your real-world impression of it - where does it fit in your treatment algorithm, what fraction of starts are still on it after a few months, and what's the main reason patients discontinue when they do? How do you counsel around the timing of administration with respect to meals to reduce side effects?

I have good results from it - but just like with any medication in psychiatry, there isn't "one size fits all". There is data that shows a more rapid titration to the highest dose (125-30) works the best. It goes 2 days with 50-20, then 5 days with 100-20, then transitions to 125-30. You have to mak...

While it's not 100% success rate, in my case, it's good to have it in your back pocket (not literally) if they don't respond to other antipsychotics, which are D2 antagonists. Obviously, psychosis isn't due to only dopamine antagonism (as widely thought)... or our regular antipsychotics would help e...
