How do you approach hippocampal sparing RT in the setting of peri-hippocampal metastases?

How close can a metastasis be to the PRV05 and still make hippocampal sparing feasible? Do you ever reduce the margin on the hippocampi to 3 mm, if you have mets closer to the hippocampus(pi)?
