How do you choose second-line treatment for patients with metastatic renal cell carcinoma after progression on anti-angiogenic therapy?

Data from recent randomized trials have shown benefit for nivolumab, cabozantinib, and lenvatinib + everolimus, but no direct comparisons exist. Are there any patient factors or other considerations that guide your choice of second-line therapy?
