How do you decide on choosing the dosing schedule of nivolumab as 240 mg q2weeks vs 480 mg q4weeks?

The Q4wk dosing is certainly easier for patients as far as scheduling goes. Is it more toxic? Should the Q4wk dosing be used only after tolerance of the Q2wk dosing is established?
