How do you decide when to add bevacizumab for patients with advanced/metastatic lung adenocarcinoma receiving first-line chemotherapy with a platinum-based doublet?

The benefit of bevacizumab in this setting is marginal, but well-supported by data. However, the risks of the drug can be substantial. How do you decide when to use it in the absence of an obvious contraindication to the drug?
