How do you decide which patients with asymptomatic hyperkalemia and CKD warrant inpatient admission versus expedited outpatient management?

What is your threshold for sending a patient with asymptomatic hyperkalemia and CKD to acute care rather than managing outpatient?

• Potassium ≥5.5 mEq/L — I have a low threshold given CKD
• Potassium ≥6.0 mEq/L — this is my threshold for ED referral or admission
• Potassium ≥6.5 mEq/L — I attempt outpatient management up to this level
• I don't use a fixed threshold — EKG findings and clinical context drive my decision regardless of level
