How do you define sepsis in 2024?

SOFA score requires arterial blood gas which is not applicable to most patients outside of the ICU. qSOFA only uses tachycardia, tachypnea, and delirium. By abandoning SIRS criteria are we missing the ability to detect early sepsis? How does this affect documentation/billing, as well?
