How do you distinguish heparin resistance from heparin effect in a patient receiving IV anticoagulation?   

Heparin ~30 units/kg/hr and is reaching aPTT goal. aPTT curve is also not erratic. 

What tests would you consider and if confirmed, would you consider ATIII concentrate or switch to an alternate IV anticoagulant?
