How do you handle medication refills for patients on traditional DMARDs who are lost to follow-up?

• Refill only if patient has a future appointment scheduled and recent labs (within past 3 months) available
• Provide a short term refill (ex. 2–4 weeks) to bridge until labs are completed and visit is scheduled
• Refill until the next scheduled appointment regardless of last labs
• Require updated labs before issuing any refill, regardless of appointment
• Do not provide refills until patient is seen in clinic and has completed labs
