How do you manage in-situ or invasive carcinoma arising within laryngeal papillomatosis?

The presence of papillomatosis can greatly complicate the ability to determine the true extent (or limit) of carcinoma. It could also greatly compromise voice quality and the chances of preserving a functional larynx. How do you factor this into your standard treatment approach for laryngeal cancer?
