Liberalize salt intake.

Use compression stockings if tolerated.

Avoid medications that can lower blood pressure.

Rise slowly.

Start Midodrine with 10 mg daily during waking hours and increase to three times a day as tolerated. Don't take it within 3-4 hours before sleep.
