How do you manage recurrent inflammatory pleural effusions in lupus pleuritis?

I'll stick to the immunomodulatory treatments and not discuss thoracentesis and pleurodesis for large and recurrent effusions. (These are rarely needed anyway, and I'd get the help of a pulmonologist).
