How do you sequence vaginal cuff brachytherapy with EBRT for post-op endometrial or cervical cases that require both modalities?
Sequentially or do you integrate the cuff brachy with the EBRT and if so what schedule, BIW, weekly, etc?

We do sequential without any break after EBRT

If providing therapy in the adjuvant setting, no recurrence at cuff, women will thank you by sequencing brachytherapy first followed by EBRT without break.

I typically perform the brachytherapy after completing the external and I deliver it on a twice weekly basis. I prefer to start the pelvic RT first as, theoretically, that is when you would have the least tumor burden in the pelvis.
