How do you treat cervical cancer with an ovarian metastasis?

Considering this is stage IV disease, do your recommendations for definitive or adjuvant treatment (after surgery) change?

This is a data free zone but several papers report a very poor prognosis for these patients. For a limited ovarian met with pelvic localized disease, one may consider chemoradiation and add adjuvant chemotherapy (to follow) but these patients generally fail systemically. If not felt to be sterilizable, can discuss minimally invasive RSO/LSO with gyn onc then definitive chemoradiation followed by chemo +/- pembrolizumab if CPS+, given the high risk of peritoneal and distant disease.
