How do you treat intact SCC of the subglottic larynx?
NCCN Guidelines do not address this rare form of H&N cancer.
What dose/fractionation would you use for definitive radiation? Is elective nodal coverage required in all cN0 cases or are there cases where you treat the larynx only?
When would you recommend addition of concurrent systemic therapy?

This is a very challenging subsite, for which most of us have less clinical experience than for glottic or supraglottic tumors, and for which the available published series are fewer and more heterogeneous.

However, I would highlight that for T1-2N0 subglottic primaries or glottic primaries extending caudally 5+mm below the cords, I would strongly consider targeting elective nodal volume...
