How often do you recommend performing an advanced lipid panel for monitoring of lipid lowering therapy?
Answer from: at Academic Institution
I am late to the responses, but I do not ever order an advanced lipid panel. Our institution does not have it on the lab menu either (one has to go to an outside lab to get it done). Anything needed for CV risk assessment can be gleaned from the history, including family history and a standard lipid...

I typically get an advanced lipoprotein profile to either help in diagnostic workup (sub-classification) of patients with hypertriglyceridemia or to help in further risk assessment of these individuals. There is limited utility of advanced testing in patients with normal triglycerides.

I do not generally recommend advanced lipid testing. My goals are primarily LDL-focused so depend on a basic fasting lipid panel and the calculated LDL, which, if the triglycerides are not severely elevated, has been shown to be more accurate than LDL by NMR or direct assays. I also pay attention to...
I don't believe advanced lipid panels add much at all. If a patient has a TG <150 is it very likely they have large/normal size LDL particles and their apoB will will about about 20% lower than their LDLC. With higher TG levels and usually low HDL, they will have more small LDL size p...
