How would you approach a male patient who has IPF s/p remote lung transplant now presenting with moderate titer ANA, Raynaud's and rapidly progressive sclerosis of the skin while on mycophenolate and tacrolimus?
Biopsy suggests sclerodermatoid GVHD but time course does not support that. ANA positive but no other antibodies to suggest SSc.
