How would you approach a patient with relapsed/refractory mantle cell lymphoma (had prior ASCT) with concurrent therapy-related MDS with excess blasts ?

Would treatment differ if transplant-eligible vs. ineligible? Would you combine BTKi with hypomethylating agent?
