How would you approach a patient with rheumatoid arthritis and pyoderma gangrenosum whose RA is non-responsive to TNF-inhibitors?

Pyoderma gangrenosum responded to TNF-i, but RA was active even with methotrexate. Tofacitinib worked for RA but pyoderma gangrenosum flared.
