How would you counsel a patient with extragenital lichens sclerosis on the acute and long term risks of post-lumpectomy radiotherapy?   

Assume a mild but diffuse case of lichens sclerosis with involvement of the ipsilateral breast. If node negative disease, would you recommend she undergo mastectomy to avoid the potential for RT-related toxicity? If the decision is to treat with adjuvant RT, would you advise standard fractionation and/or partial breast irradiation?
