How would you manage an elderly patient with a de novo TNBC in the setting of a remote prior ipsilateral breast cancer s/p mastectomy and implant?  

The patient is greater than 70 years old with a new de novo, 4 mm triple-negative breast cancer.

She has a previous history of ipsilateral breast cancer treated with mastectomy and breast implant in the distant past (30 years ago) and no history of previous radiation therapy.
