How would you manage and prevent radiation recall?

If a patient has had severe radiation recall reaction in the past, but was successfully restarted on the precipitating chemotherapy agent, what steps would you take to avoid another reaction while receiving radiation at a second site? Would your choice of radiation dose or fractionation change? Would you use IMRT to decrease skin dose? What would be your preferred interval of time before chemotherapy after radiation?
