How would you treat a patient with anal SCC if radiation is contraindicated?

A patient's kidney transplant allograft would be in the radiation field and cannot be moved. Would you recommend surgery (i.e. APR) or still try for some type of chemo-radiation with consideration of brachytherapy? How has this approach changed with arrival of IO such as Pembrolizumab?
