How would you treat a patient with isolated CNS relapse of seminoma?
If CNS lesions were managed with surgery/radiation, is systemic therapy also warranted? If so, would you preferentially recommend 2nd line high dose chemotherapy or what clinical data would you use to inform that decision?

As one would expect there are really no reliable data to use to make decisions. I am assuming that there is not an significant elevation of HCG or AFP?.
