In a patient with chemotherapy induced anemia, with other causes ruled out, do you favor transfusion or ESA support?

Regarding ESA support, there seems to be ongoing concerns about increased mortality and thrombosis risk. In some studies the increased risk seemed to be associated with using a goal Hgb >12; however, if using a lower goal, some studies suggest no increased risk.
