I would definitely treat with that level of fibrosis. Sometimes, discussing the dangers of progressing fibrosis to cirrhosis without treatment will put the fear of God into them and get them to change their lifestyle.
I would still do it because options are otherwise limited. I may consider GLP-1 more so to affect weight loss if someone is struggling with lifestyle changes. The current approach I am using is using RES for at least 1 year if all is tolerated well. Lifestyle changes would be a plus but not a barrie...
