In patients with recurrent ischemic infarcts on the same side as an internal carotid artery lesion, is intervention warranted even if there is less than 50% obstruction?

When would you consider stenting or CEA even if there is less than 50 obstruction when no other etiology has been found?

If a carotid web is found, it would be reasonable to consider intervention to prevent thrombus formation and embolization.
