Is there a role for ipilumumab and nivolumab combination immunotherapy or single agent ipilumumab for maintenance after induction with combination immunotherapy in metastatic melanoma? If so what is the schedule and dosing?

A patient with metastatic melanoma who progressed on nivolumab after 40 cycles now has a dramatic response to combination ipilumumab and nivolumab. After 4 cycles of induction with this regimen, what would be the desired maintenance regimen? Most maintenance regimens use nivolumab but this patient has previously progressed on it.
