This is an important question, as pill burden does have a negative effect on the quality of life for the patient as well as the care provider. This becomes even more important with patients having dysphagia and or loss of appetite, common symptoms in patients with advanced dementia.

In general, I think about cholinesterase inhibitors as most appropriate for early to moderate AD and as Namenda most appropriate for moderate to severe stages. So, when I start cholinesterase inhibitors, I set the expectations that we will, most likely, plan to stop them at some time.
