What is your preferred formulation of parenteral iron?

There are various formulations of intravenous iron; each with varying costs, test dose requirement, elemental dose, and number or time of infusions needed. How do you approach selecting the optimal formulation of intravenous iron and/or do you prefer certain ones in specific clinical scenarios (e.g. heart failure, CKD, pregnancy etc.)?

The question is not simple. The formulation I use the most is ferumoxytol because four insurance carriers allow a total dose infusion of 1020 mg in 20-30 minutes.
