What is your simulation setup when treating bladder with concurrent chemoradiation?
Assume treating to 64-66 Gy.
Do you simulate bladder cancer patients with full and/or empty bladder?
Do you ever add your own delayed IV contrast to illuminate bladder tumor for boost contouring similar to a CT IVP?
Do you simulate boosts any differently?

For the majority of our MIBC patients, we do the planning CT in the supine position with arms on the chest. In select cases with concern for small bowel dose, we will simulate in the prone position if safe and comfortable for the patient.
