What molecular testing are you ordering in the adjuvant and neoadjuvant setting for GI cancers?

Do you routinely do universal NGS testing? Or only order MMR and pik3ca in colon cancer?

The answer depends on the specific GI cancer and the clinical setting (resectable vs. metastatic). Universal NGS is not recommended for resectable non-metastatic colon cancer. In colon cancer, I order MMR by ICH and MSI by NGS. I also order PIK3CA, etc., testing. In pancreatic cancer, I often get NGS testing given the high chance of recurrence.
