We usually do shave removal, excision or punch excision and send for pathology.

For very pigmented lesions, sometimes, I used the 100 µs mode with the alexandrite laser, particularly for small 1 to 2 mm junctional nevi.
For larger lesions, I typically use the erbium YAG laser or CO2 laser and ablate the lesion until it is flush with the skin.

Fully ablative CO2 laser under the appropriate settings leads to complete removal and no scar. This is regularly performed in South Korea for benign melanocytic nevi.
