What volumes do you cover when delivering salvage radiation for a contralateral neck recurrence after upfront ipsilateral neck radiation for tonsillar cancer?
Do you treat just gross disease with margin or electively treat levels Ib-V? What if the patient is elderly and there is a pacemaker in level IV?

Levels 1b, 2, 3, 4, 5, and RP on the previously unirradiated neck.

I'm not sure on the routine clinical need of radiating contralateral IB, or V if it's an isolated neck recurrence - depends on if you would've electively treated that if the patient had not had surgery. II-IV I agree with.
