When do you recommend plasmapheresis/plasma exchange for management of severe hypertriglyceridemia?

Plasmapheresis may increase risk of infection. IV insulin drip and oral medications are helpful. I don’t know if treating hypertriglyceridemia faster with plasmapheresis will change the outcome.
