Would you offer adjuvant radiation to a patient with uterine undifferentiated sarcoma s/p resection, vaginal cuff recurrence, and re-resection?

If so, would you treat the whole pelvis, cuff brachytherapy, or both?

Following principles of managing sarcoma, I would favor RT with a combination of EBRT and brachy.

I agree with XRT as per @Dr. First Last. This patient's tumor has clearly shown a predisposition for local recurrence and often large margins are not attainable. I would also favor EBRT plus brachy. You may also likely consider systemic staging if not already performed.
