That nodal region has previously received 55 Gy in 25 fractions.

If no gross disease and has had previous RT, I would favor no additional RT.

Depending on patient motivation and any other relevant clinical factors, I would consider chemoIO, especially if not treated with IO during chemoRT, as relapsed/persistent patients were included in KEYNOTE-826.
