Would you pursue neoadjuvant therapy or upfront surgery in a postmenopausal patient with non-inflammatory cT4b N1 HR+ breast cancer?

In setting of cT4b (skin thickening around nipple but not meeting criteria for inflammatory) N1 M0, grade 1, ER 95%, PR 70%, Her2 negative, ki67 5%, would you pursue upfront surgery or consider neoadjuvant chemo or endocrine therapy? Surgery thinks breast conservation is possible however she would need ALND for that positive axillary node. With this biology, however, patient may not have much response to neoadjuvant chemotherapy and still end up with ALND.
